I'm currently attending a class about Medical Office Procedures, but my teacher sucks. She gave us 5 weeks to complete all the assignments and she doesn't help us unless we're in class. I have a total of 87 assignments and I'm currently stuck on the health insurance forms. The area to the left of the form, 1a. - 13a, has me confused.
Anyone with claims experience that can help?

Hi there, I am a medical biller and I do claims all day long. I can help you with this but you’ve to tell me what form your using. UB04 or Claim 1500? then I have the ability to answer yoy better.
